Nominations will be accepted for efforts involved with minorities in regard to race, color, creed, gender, sexual orientation, national origin, or disability.
The recipient should have made outstanding contributions to human and civil rights and meet one or more of the following criteria:
a. Expanding educational opportunities for minority students or students with disability;
b. Advancing the cause of human and civil rights within public schools and/or the community;
c. Promoting health and safety within public schools and/or the community.
Supporting documentation
Nominations must include several photos of the nominee involved in their award-winning activity. All photographs must be in color. Electronic photo files (jpg or png - hi-res, 300 dpi) and/or photographs, minimum 4 x 6 size are preferred. Other photos of the nominee(s) engaged in activities (i.e., a teacher interacting with students) are acceptable. Photos do not have to be done professionally.
